Leak Thru Bomar Hatch Lever/Dog

Was fiddling around the boat this morning since it was raining. Yes, I know hard to believe it was raining in the Midwest
Anyway, moved some pillows around and noticed one was wet. Looked up and saw drips coming from a corner of the Bomar hatch.
Upon closer inspection I believe it's coming through the hole drilled for the hatch dog - and not the actual seal of the hatch body - it then accumulated on the shade and dripped out of the corner.
I used a Phillips screwdriver to disassemble the lever and saw one O-ring on the top side - and all looked good as far as I could tell. Reassembled and tightened it down. Tight enough to still turn the lever, but not too loose to leak (I hope).
Anyone else have this issue in the past? Is this the standard? Or am I missing anything? Is this another item to add to the checklist to make sure it's tight (but not too tight)?
