I have seen several questions about this asked on here, and from what I have read, nobody has posted this source. I think it makes it very simple to diagnose with this one!

    It's inside the 6th thread down at the top of the page. It's a pinned thread. Under "Encountering Faults"  Good to have a reminder though....
