2006 Volvo 5.7 GXI-FF head damage plus plus

This damage is hidden under the exhaust manifold on right side of the motor. Had no clue it was there,  only found it when I could not get that spark plug out for normal tune up. What caused it ?. Motor runs fine right now. This is obviously a major issue. Head damage possible block too. Any Ideas were to start ? Thanks
